description ABSTRACT Antisperm antibody (ASA) is one of the immunological factors that play a role in unexplained infertility. About 10-30% of infertile couples are caused by ASA. ASA is thought to be lowered by paternal lymphocyte immunization (PLI), but the immune mechanism that occurs in the body is unclear.  This research aims to understand the immune mechanism after PLI in women with unexplained infertility by analyzed ASA, IFNγ, IDO, and Treg cells populations. This research using observational analysis, conducted in X Jakarta Hospital from June 2018 to April 2019. Samples were 12 unexplained infertility women with ASA>1:128. Samples were examined ASA titers using the husband's sperm auto-agglutination test (HSAaT) method. Tregs were evaluated using flow cytometry. IFNγ and IDO was determined using an ELISA. The data were analyzed using the Mann-Whitney and Spearman tests. Results show that PLI can reduce ASA. Treg cells percentage ratio increased after 3 times PLI by 20% and 6 times PLI by 50%, but the increase was not significant.  An increase of high IFNγ is followed by an increase in the IDO ratio post/early research and an increase in the percentage of the Treg cell population.  The results of this study can be considered as a diagnostic use as well as one of the benchmarks in identifying unexplained causes of infertility, IVF failure, and increasing the success of IVF, can also be used as a prognostic for pregnancies with a history of recurrent miscarriage so that subsequent pregnancies are better. ABSTRAK Antibodi antisperma (ASA) merupakan salah satu faktor imunologi yang berperan pada infertilitas yang tidak terjelaskan. Sekitar 10-30% pasangan infertil disebabkan oleh ASA. ASA diduga dapat diturunkan dengan paternal lymphocyte immunization (PLI), namun mekanisme imun yang terjadi di dalam tubuh belum jelas.  Penelitian ini bertujuan untuk mengetahui mekanisme imun pada perempuan infertilitas yang tidak terjelaskan setelah diberikan PLI melalui analisis ASA, IFNγ, IDO, dan populasi sel Treg.  Desain penelitian adalah observasional analitik.  Penelitian dilakukan di RS X Jakarta dari Juni 2018 sampai dengan April 2019. Sampel sejumlah 12 perempuan infertilitas tidak terjelaskan dengan ASA >1:128. Pengukuran ASA menggunakan metode husband's sperm auto-agglutination test (HSAaT).  Populasi sel Treg diukur dengan flowsitometer.  IFNγ dan IDO diukur menggunakan ELISA. Data dianalisis menggunakan uji Mann-Whitney dan Spearman. Hasil penelitian didapatkan bahwa PLI dapat menurunkan ASA. Rasio persentase sel Treg meningkat setelah 3 kali PLI sebesar 20% dan 6 kali PLI sebesar 50%, namun kenaikan tesebut tidak bermakna. Kenaikan IFNγ yang tinggi diikuti oleh kenaikan rasio IDO post/pre dan kenaikan persentase populasi sel Treg. Hasil penelitian ini dapat dipertimbangkan sebagai penggunaan diagnostik, salah satu tolok ukur dalam mengidentifikasi penyebab infertilitas yang tidak terjelaskan, meningkatkan keberhasilan IVF, serta digunakan sebagai prognostik kehamilan dengan riwayat keguguran berulang agar kehamilan berikutnya menjadi lebih baik.
